Elicere Health Partners
"Improving health and wellness one loved one at a time."
We Don’t Consult. We Execute. Elicere Health Partners is a healthcare performance operating company built to turn unrealized potential into measurable performance. We uncover where healthcare organizations are losing capacity, revenue, efficiency, and patient access, then put the people, technology, workflows, and operating infrastructure in place to fix it. We don’t stop at recommendations. We work inside the operation, measure what matters, execute alongside your team, and remain accountable until performance improves. Find the friction. Fix the system. Measure the results.

Dr. David Pak, DO
Founder & Chief Executive Officer
Dr. David Pak is a board-certified Internal Medicine physician, healthcare executive, and entrepreneur with more than two decades of experience building, leading, and transforming physician organizations. Throughout his career, he has combined clinical expertise with operational leadership to help healthcare organizations improve patient access, enhance provider productivity, strengthen financial performance, and deliver higher-quality care. Driven by a passion to improve healthcare from both the bedside and the boardroom, Dr. Pak founded Elicere Health Partners after recognizing a recurring challenge throughout the industry: healthcare organizations often know what needs to change but lack the operational infrastructure to execute lasting transformation. Elicere was created to bridge that gap by combining physician leadership, operational excellence, artificial intelligence, and scalable virtual clinical teams to help organizations achieve measurable, sustainable results. Unlike many healthcare consultants, Dr. Pak has spent his career operating physician practices from the inside. He has founded, acquired, integrated, and managed multi-specialty medical organizations while leading teams through periods of rapid growth, operational transformation, and evolving reimbursement models. His experience spans independent physician practices, multi-site medical groups, value-based care organizations, behavioral health integration, post-acute care, virtual clinical operations, and healthcare technology implementation. Throughout his career, Dr. Pak has remained committed to delivering exceptional patient care. He is especially proud to have been recognized by the Comal County community newspaper Herald Zeitung as Practicing Physician of the Year for four consecutive years, an honor that reflects the trust of his patients and the lasting relationships he has built within the communities he serves. As Founder and Chief Executive Officer of Elicere Health Partners, Dr. Pak believes healthcare organizations do not need another consulting firm, they need an execution partner. Elicere works alongside physician practices, health systems, investors, and healthcare organizations to identify operational constraints, implement scalable solutions, and build sustainable infrastructure that improves both clinical and financial performance. Dr. Pak’s leadership philosophy is rooted in execution. While many organizations can identify problems, few possess the operational discipline required to solve them at scale. Elicere was built to transform strategic recommendations into measurable operational outcomes. His work has consistently focused on physician workflow optimization, behavioral health integration, chronic disease management, AI-enabled clinical operations, virtual workforce development, and healthcare infrastructure modernization. He believes the future of healthcare lies in empowering clinicians through technology, not replacing them, allowing providers to spend more time with patients while improving access, reducing burnout, and strengthening organizational performance. Today, Dr. Pak partners with physician leaders, health systems, healthcare organizations, and strategic investors to build scalable, technology-enabled healthcare platforms that improve patient outcomes while creating lasting enterprise value. His mission is simple: help healthcare organizations unlock their full potential so providers can focus on delivering exceptional care. Carolina Pavese
Chief Operating Officer
Carolina Pavese is the Chief Operating Officer at Elicere, where she partners with healthcare organizations to bring clarity to complexity. As a registered nurse, healthcare executive, and organizational strategist, she brings more than two decades of experience helping leaders align people, systems, and strategy to build resilient organizations that deliver exceptional care and sustainable performance. Her perspective is rooted in her career as a registered nurse. Early in her career, Carolina witnessed both the extraordinary dedication of healthcare professionals and the challenges they faced every day. She saw talented clinicians, leaders, and care teams deeply committed to delivering exceptional care while navigating fragmented systems, competing priorities, and organizational barriers that too often made their work harder than it needed to be. Those experiences shaped her belief that exceptional healthcare depends not only on clinical expertise, but also on intentional leadership, thoughtfully designed organizations, and systems that enable people to do their best work. At Iora Health, Carolina found an opportunity to put that belief into practice, helping build one of the country's pioneering relationship-based primary care models, which ultimately exited to One Medical and is now part of Amazon. Since then, she has served in executive leadership roles across health systems, value-based care organizations, and venture-backed healthcare companies, partnering with founders, executive teams, and frontline leaders to strengthen leadership, improve operations, redesign care models, and build organizations equipped for growth, innovation, and lasting impact. Today, Carolina partners with healthcare organizations navigating growth, transformation, or operational complexity. Whether the need is leadership development, organizational alignment, workforce strategy, care model redesign, operational improvement, scalable infrastructure, or AI-enabled transformation, she helps leaders identify what matters most and build practical, sustainable solutions that strengthen both people and performance. Carolina believes the strongest healthcare organizations understand that people and performance go hand in hand. Exceptional patient care, thriving care teams, confident leaders, operational excellence, and financial sustainability are not separate goals—they are the natural outcome of organizations intentionally designed to bring out the best in people.
Tomasa O'Hern
Senior VP of Operations
With more than 34 years of experience in revenue cycle management and operations across Primary Care, Ophthalmology, and Dermatology, Tomasa O’Hern is a healthcare operations executive who has built her career at the intersection of clinical operations and financial performance. Beyond her core specialties, Tomasa’s expertise extends across a broad range of clinical areas, including internal medicine, optometry, behavioral health, gynecology, anesthesiology, rheumatology, neurology, podiatry, general surgery, infusion, and rural health. An entrepreneur since 1998, Tomasa built her career alongside her husband, Jerry O’Hern, launching and co-owning multiple businesses across several sectors of the market. Her path into healthcare began in 1992, when she went to work for an ophthalmologist in her hometown of Kerrville, Texas, an experience that sparked a lasting passion for the medical field. That passion led her to found her first company, O’Hern Medical Billing (OMB), a full turnkey revenue cycle management firm specializing in primary care, at the age of 21 in San Antonio, Texas, where she served as President from 2000 to 2015. Over the 15 years she led her RCM company, Tomasa also co-owned businesses in IT, project management, and EMR technology. In 2011, she moved back to Kerrville, Texas, to be closer to family, where she went on to open her first restaurant, Yeo-Bo’s Cafe Korean BBQ (2011–2023), and later built a chain of three brick-and-mortar clothing boutiques under Tomé Boutique (2011–2022), both ventures she successfully grew and sold. In December 2021, Tomasa was recruited back into the medical field, joining Pak Medical Group (which later joined InnovaCare Health). Since then, she has advanced through a series of progressively senior roles. Outside of work, Tomasa cherishes quality time with friends and family. Travel and sushi are among her greatest passions.
Reagan Lopez
Director of Strategy & Integration
Reagan Lopez leads strategy and operational buildout at Elicere Health Partners, working alongside the executive team to bring new programs from concept to launch. Her focus spans client evaluation, program design, implementation, and the day-to-day infrastructure needed to support Elicere’s growing portfolio of healthcare partners. With a hands-on approach to strategy and integration, Reagan helps ensure that new initiatives are not only thoughtfully designed, but also practical, scalable, and aligned with the needs of each organization. Reagan came to Elicere from ELEOS Healthcare Solutions, where she spent four years as Integration Manager overseeing Virtual Health Associate staffing and operations across a diverse portfolio of medical group clients. Her responsibilities extended throughout the client lifecycle, from onboarding and credentialing to workforce coordination, implementation, and long-term client relationships. Working closely with providers, clinical teams, and operational leaders gave her a firsthand understanding of the challenges healthcare organizations face when introducing new programs, workflows, and support models into already busy practices. That experience shaped Reagan’s approach at Elicere today. She believes successful healthcare transformation begins with understanding how an organization actually operates, not simply how it appears on paper. By evaluating workflows, identifying operational gaps, and listening to the people doing the work every day, she helps develop solutions that strengthen existing teams rather than disrupt them. In her role as Director of Strategy & Integration, Reagan serves as a bridge between strategy and execution, helping turn organizational goals into clear processes, sustainable workflows, and measurable action. Her work is centered on building strong client partnerships, creating operational clarity, and ensuring Elicere’s programs deliver meaningful, lasting value for the healthcare organizations and teams they support.
